Insurance and Superbill
At Vida Family Medicine, we do not accept insurance. Our goal is to provide a high level of care without interference from insurance companies. This allows us to spend more time getting to know you and focus on your individual needs.
Instead of billing insurance, we provide transparent pricing for our services. We can see patients with any insurance carrier as well as those who are uninsured. We do provide superbills, for patients to submit directly with insurance for potential out-of-network reimbursement.
Important Note: We do not bill Medicare or Medicaid, and due to federal and state regulations, we are currently unable to offer services to patients eligible for Medicare or Medicaid. This may change in the future. If you have Medicare or Medicaid and would like to become a patient, please email us to be added to our waiting list.
What is a Superbill?
A superbill is a detailed receipt of your visit that includes all the information your insurance company needs to process a reimbursement claim.
Each superbill includes:
Your name and date of birth
Provider information and credentials
Date(s) of service
CPT (procedure) codes
ICD-10 (diagnosis) codes
Amount paid
How Do I Find Out About Reimbursement with Insurance?
Call your insurance company’s Member Services line and ask about:
Out-of-Network Coverage: Does my plan cover visits with out-of-network providers?
Deductible & Coinsurance: Do I have a deductible? If so, have I met that deductible? What percentage of each visit is covered?
Submission Process: How do I submit Superbills (mail, fax, or online portal)?
Timely Filing Limits: What is the deadline for submitting Superbills after each visit?
Prior Authorization: Does my plan require approval before receiving these services?
Additional information needed to inquire about reimbursement with insurance:
Initial Intake Package: Includes two visits for $650 total
Billing E/M codes: 99205 (first visit) and 99214 (second visit)
Follow-Up Visits: $249 each
Billing E/M code: 99214
Helpful tip when calling insurance: Always write down the reference number for your call along with the name of the rep you spoke with.
How Do I Submit My Superbill?
Pay for your visit upfront at the time of the visit.
Submit your superbill using their preferred method:
Online portal, or Mail/email with a claim form
Wait for your insurance company to process your claim. Reimbursement is sent directly to you.

Keep copies of all superbills and receipts.
Reimbursement amounts vary depending on your plan and deductible.

Yes! You can use an FSA or HSA card for payment.

Superbills are sent in batches once per month for the previous month’s visits. For example: Appointments in January → Superbills sent by the second week of February

Reimbursement depends on your individual plan’s out-of-network benefits, deductible, and coverage level. We recommend contacting your insurance provider for details. We cannot guarantee reimbursement.
